Turkey” isn’t just at the dinner table!

As I was thinking through the meal I’ll help prepare next week for my family, I started thinking about how we sometimes become the “turkey” in our workplaces. A turkey is wonderful bird, very large when left to grow in the wild. However, most of the fouls we actually eat are raised on a farm, and never get to experience the freedom, and the fear, of being wild.

When I think about my work history, I found myself doing the same thing. I had the desire to be free, but was getting caught in the ‘corporate farm’. If you’re a hunter, you know there are a couple different ways to raise animals. You can do the farm and slaughter them when it’s time: it’s efficient and cost-effective. However, you can also have a “reserve”, where you give the animals a safe, controlled environment to grow, but it’s open, more like being in the wild, but with boundaries to keep them safe. Hunters can then come to hunt in a limited space.

Healthcare Options…what’s best?

Over the weekend, the U.S. House of Representatives passed a Healthcare bill 220-215. So we’re one step closer to the healthcare overhaul President Obama promised during the campaign season prior to his Presidency.

As this bill heads to the U.S. Senate for debate and vote, I can’t help but wonder if this is really a great option for our country, our businesses, and our Healthcare Providers. Below is an article I found from the Boston Globe that made a lot of sense to me, and I hope it will to you as well.


An option for public: less government, more choice

“My guiding principle is and always has been that consumers do better when there is choice and competition.’’ So said President Obama in his address to Congress on health care, making an argument for a government-run “public option’’ to sell health insurance that many Democrats have echoed.

In 34 states, Obama noted, three-fourths of the insurance market is controlled by five or fewer companies. “Without competition, the price of insurance goes up and the quality goes down.’’ But add a public option “administered by the government just like Medicaid or Medicare,’’ he said, and competition would revive.

No, it wouldn’t.

A government-run health insurer would radically tilt the health-insurance playing field. It would amount to a new entitlement program, able to undercut the price of private insurance by squeezing hospitals and doctors, reimbursing them at below-market rates. “Just like Medicaid and Medicare,’’ which also underpay medical providers, the public option would force hospitals and doctors to charge private insurers more. Insurers would be compelled to raise their premiums, eventually losing millions of customers to the government plan.

Health Care…Interrupted

I read this article this morning at the Herald Tribune about the impact of our care providers being interrupted during office visits. With all the changes on the horizon with EHR and increased patient coverage, will these interruptions continue to increase?

Interruptions, in any environment, are inevitable. We need to extend each other the courtesy of our time and attention when our paths do cross.

“Now, where were we?” I find myself saying that all too often in my practice these days, after a visit is interrupted — by a nurse, another doctor, a laboratory or a pharmacy. Whatever the reason, I am often forced leave the room to resolve the issue. Then I return and try to pick up where I left off. For me, it’s a distraction; for the parent of a sick child, it can be alienating and infuriating.

And it’s not just my patients and me. In a 2001 study published in Family Medicine, residents were observed during 60 primary-care office visits that lasted an average of 11 minutes. Each doctor, on average, was interrupted twice.

The Hidden Victims of Health reform

President Barack Obama’s health-care sales pitch depends on his ability to obfuscate, confuse, those likely to get hurt by reform.

At this past Wednesday’s news conference, for example, he was asked “specifically what kind of pain and sacrifice” he would ask of patients in order to achieve the cost savings he promises. He insisted he “won’t reduce Medicare benefits” but instead would “make delivery more efficient.” The most Mr. Obama would concede is that some people will have to “give up paying for things that don’t make you healthier.” That is simply not credible.

A HEALTH-TECH MONOPOLY

Democrats are using the economic “stimulus” bill to create a health information monopoly that will help centralize government control of the health-care market.

In theory, electronic medical records are among the few stimulus ideas that might do some actual good. Democrats and Republicans agree that exchanging the paper files we mostly use now for digital versions will lower costs and cut down on medical errors:

Both the House and Senate stimulus bills include about $20 billion in incentive payments (mainly through Medicare and Medicaid) to encourage the digitization of medical records.
